CURRENT METHODS OF REPAIRING CARTILAGE DEFECTS

In order to overcome the drawbacks inherent in traditional methods, alternative methods have been developed: osteochondral or chondrocyte allografts and autografts

ในสมัยก่อนมีการรักษาหลายแบบแต่ตอนนี้เราภูมิใจเสนอการรักาาแบบใหม่ที่ไดผลดีและมีประสิทธิภาพโดยสามารถใช้ chondrocyte allografts and autografts ก็ได้

แปดสัปดาห์ จะเห็นว่ารอยแทบหายไปเลยครับ

SURGICAL TECHNIQUE

เราต้องจัดการตัดแต่งผิวข้อที่เสียหายให้ดูดีก่อนนะครับ อย่าให้มีผิวที่ยังหลุดร่อนยังคงอยู่นะครับเพราะจะทำให้การแก้ปัญหาไม่หมดจะกลับมาเป็นอีกครับ




วางแผนเจาะรูว่าจะเจาะกี่่รูและความลึกให้แน่ใจว่าลึกประมาณ ๑๐ ถึง ๑๕ mm เพราะต้องเจาะจนพบ Vascular

Take Graft จากจุด Non Weight Bearing โดยมากนิยม Anterior Condyle โดยจะต้องเล็งทิศทางให้ดีตรงกับพื้นผิวของ Cartilage ด้วยนะครับ

เราสามารถตอกเพื่อ Take Graft ได้หลายความลึกโดยมากเราจะตอกที่ความลึก ๑๕ ถึง ๒๐ มม

เพื่่อทำการตัดแต่ง Graft ให้ได้ความยาว ๑๐ ถึง ๑๕ มม

จากนั้นให้ใช้ Harvesting Tamp ขนาด 4.5 mm สอดเพื่อให้ดึง Graft ออกมาได้ง่ายอาจต้องมีการโยกและหมุนบิดบ้าง

จากนั้นใช้ Chisel Guard ประกอบเข้ากับ Harvesting Chisel แล้วสอดเพื่อนำ Graft ออกมา โดยห้ามกระแทกแรง เพราะจะทำให้ Cartilage ตายได้ (อิบหาย)

อปกรณ์ในรูปสามารถ Calibration  ได้โดยการไขให้สั้นหรือยาวได้โดยการไขให้สั้นจะทำให้ Graft เข้าไม่ลึกมาก  แต่ในทางตรงกันข้าม ถ้าไขให้ยาวออกมาก็จะทำให้ Graft เข้าไปลึก ซึ่งประเด็นนี้ต้องอยู่ในดุลยพินิจของแพทย์ที่เคารพครับ ห้ามตัดสินใจเอง แต่ถ้าถามผมผมจะบอกว่าให้ Graft เสมอกันกับของเดิม

ขณะใส่ Graft จะต้องระวังเรื่องทิศทางของการใส่ Graft ถ้าใส่ผิดด้าน ก็หมดกัน

อีกข้อควรระวังในการใส่ Graft ใน Case Arthroscope ก็คือ  นำ้นั้นมีแรงดันที่สามารถทำให้ Graft หล่นลงพื้นได้ทันที และแพทย์ท่านอาจจะควันออกหูได้

เล็งด้วย Delivery แล้วใช้ Dilator ตามเพื่อหาทิสทางที่ถูกต้อง

ค่อยๆตอกเบาๆ

เสร็จแล้วก็จะสวยงามดังภาพ

อันนี้เป็นรูป พท ที่แรเงาคือ พท ที่สามารถ ทำ Mosaicplasty ผ่าน scope ได้

ในขณะที่ใส่ Graft เราสามารถมองผ่าน Slot ได้ด้วย

Exploration ก็สามารถทำได้นะครับ

ข้อเท้าก็ สามารถครับ

 

  นอกจากการรักษาด้วย วิธี Mosaicplasty Technique

ใช้ Micro Fracture ก็ได้ครับ คือใช้ Pin หรือเครื่องมือที่ออกแบบมาเพื่อการนี้โดยเฉพาะ
